Provides housing, case management, and nutrition services to Hawaii residents on Island of Oahu with disabilities, including HIV/AIDS and have very low income. Housing services include transitional shelter, short-term rent assistance, utility assistance, security deposit assistance, and rental subsidy programs. Case management is provided in conjunction with housing assistance. Nutritional services includes grocery distribution, home delivery to the house bound, and twice weekly hot lunches.

Eligibility

1) HIV positive as verified by a Hawaii-licensed physician (for HIV-specific programs). 2) Hawaii state residency. 3) Homeless or at-risk of becoming homeless 4) Very low-income as determined by Housing & Urban Development (HUD). 5) Case Management representation. 6) Able and willing to follow housing rules and guidelines as stated in the rental agreement.
