(HHH SNAP Outreach Program, Supplemental Nutritional Assistance Program Outreach) SNAP Outreach Program provides assistance to at-risk, vulnerable, and low-income individuals and households with accessing SNAP benefits. The Supplemental Nutrition Assistance Program (SNAP) Outreach is a federally funded program administered by the State of Hawaii to reduce food insecurity and help families buy nutritious food. The assistance provided includes: -help with filling out the SNAP application for the Department of Human Services (DHS) -providing interpretive assistance to Limited English Proficient (LEP) individuals -facilitating submission of the application to the DHS offices -assisting with follow-up on application status as needed -helping SNAP benefit recipients to maintain those benefits -participating with DHS-contracted providers to conduct SNAP nutrition educational activities designed to ensure proper use of the SNAP benefit, including how to use the benefit and still be able to eat nutritious meals. Applicants may be eligible for SNAP if their household income meets Federal Income Guidelines. If the household appears to meet DHS eligibility requirements, they will be scheduled for an in-person interview with a SNAP outreach worker. Anyone who is interested in applying for SNAP benefits may call for more details. There will be a short pre-screening intake over the telephone (usually takes 10-15 minutes). Our staff will schedule an appointment with you and help you complete the form. Program is also offered in the community or at the applicant's home (by appointment) as needed.
