Susannah Wesley Community Center (SWCC) operates a multi-service center with services and programs for youth, families, individuals, newcomers, senior citizens, and other residents of Kalihi-Palama. Hui Kokua or SWCC's Emergency Food Pantry is available, please call for more information. The agency has a wide range of Youth and Family Services (e.g. truancy prevention, life skills development, CBase Diploma Program, non-school hours activities, home based parenting and a juvenile assessment center). The Kalihi-Palama Juvenile Assessment Center is a juvenile justice diversion program that works with youth and their families. SWCC's multi-lingual behavioral health care coordination program services adults struggling with mental illness. The Trafficking Victim Assistance Program (TVAP), Hawaii provides crisis response and comprehensive and trauma informed case management services to minors and adults who are suspected or identified as victims of human trafficking.

(Ka Puuwai O Na Keiki, Enhanced Healthy Start, DHS Home Visiting Services) We are a team of Home Visitors that respond to referrals from CWS for Maui County and also FSS and VCM for the island of Maui, and then offer screening for risk factors, assessments, treatment planning and service delivery to reach goals of reunification, parenting skills, support, resources and referrals. Each family referred to our team will be getting visits from a Registered Nurse to identify any developmental challenges and help with health needs. She is also a lactation specialist. They will also receive visits from a Clinical Supervisor for assistance in life with things like addiction, domestic violence and mental health. And each family will be assigned a Family Support Worker who will offer Nurturing Parenting lessons and offering support in an effort to identify and decrease risk factors as well as increasing protective factors. Ka Pu'uwai O Na Keiki never has a waitlist but is specifically referral only for parents and foster parents with a child between the ages of 0 to 3. Accepts collect calls.

(PAT) Increase Your Keiki’s Opportunity for School Success! Program for families of prenatal moms, infants and toddlers called “Parents As Teachers,” or PAT - a home-based program for families that begins before your keiki is born until a keiki turns three. Through home visits, we support parents with practical, research-based strategies and activities to support the importance of enhancing school readiness during these critical and formative years of a keiki’s life. These strategies enhance parent child interactions, strengthen ohana relationships and support parents in being their keiki’s first and most influential teacher. Literacy is a major focus of PAT, books are used in every visit. The “Parents as Teachers” Foundational Curriculum is provided through supportive certified PAT parent educator’s home visits and screenings in assisting parents in understanding the latest neuroscience research and how it relates to their keiki. Ulupono Family Strengthening Program works to support families to “prosper on the right path” and increase the internal resources of families to handle challenges they face with their children. Family Support Specialists on Oahu work with parents/caregivers of children ages birth to 17 years old to: • Provide parenting education • Promote child health, development, and safety • Offer emotional support • Link families to resources in their community Families are referred to the program by Child Welfare Services (CWS) or by Voluntary Case Management (VCM) programs. Families receive short-term home visiting services. These services are voluntary. There is no charge for these services.

The program service provides voluntary screening, assessment, and referrals to Pregnant Women, Expectant Fathers, and Caregivers of Newborns residing on the island of Kauai who meet the program eligibility criteria. Healthy Start provides home visiting services to eligible families. Serves environmentally at-risk families. Participants include prenatal women and families with children birth to three years old. The infants and children are considered at-risk for child abuse and neglect and developmental delays. The program provides comprehensive home visitation, child development screening and intervention, as well as psychosocial assessment, and motivational readiness for treatment and brief counseling. The intensity of the program varies over the course of service, starting with weekly home visits, and reducing the frequency as the family and child meet their goals.
