Susannah Wesley Community Center (SWCC) operates a multi-service center with services and programs for youth, families, individuals, newcomers, senior citizens, and other residents of Kalihi-Palama. Hui Kokua or SWCC's Emergency Food Pantry is available, please call for more information. The agency has a wide range of Youth and Family Services (e.g. truancy prevention, life skills development, CBase Diploma Program, non-school hours activities, home based parenting and a juvenile assessment center). The Kalihi-Palama Juvenile Assessment Center is a juvenile justice diversion program that works with youth and their families. SWCC's multi-lingual behavioral health care coordination program services adults struggling with mental illness. The Trafficking Victim Assistance Program (TVAP), Hawaii provides crisis response and comprehensive and trauma informed case management services to minors and adults who are suspected or identified as victims of human trafficking.

(WIN Kauai) We provide clean and sober housing, bridge to success transitional housing, resource referrals, parenting classes, life skills classes, substance abuse relapse prevention, intensive outpatient substance abuse treatment, re-entry from incarceration, domestic violence education, employment services, case management, and advocacy. WIN is committed to guiding women and teens towards sobriety, self-esteem and responsibility through our warm, nurturing and holistic approach. We help our clients develop positive life skills they need to become independent and productive members of our community. We do everything we can to break the generational cycle of hopelessness and abuse. WIN gives preference for admission of pregnant women in our housing and treatment program.

Services include assessment, safety plan, individual service plan, case management, domestic violence information, counseling and referrals.

Provides housing, case management, and nutrition services to Hawaii residents on Island of Oahu with disabilities, including HIV/AIDS and have very low income. Housing services include transitional shelter, short-term rent assistance, utility assistance, security deposit assistance, and rental subsidy programs. Case management is provided in conjunction with housing assistance. Nutritional services includes grocery distribution, home delivery to the house bound, and twice weekly hot lunches.

These issues negatively affect youths' ability to successfully live at home or remain in school and they interfere with positive productive family and peer relationships. Two evidence-based services are provided through our ISS program on Kauai. -Multi-systemic Therapy (MST) is an evidence-based treatment program for youth with serious and complex problems who live at home. Therapists work with youth and their parents on specific goals that will enable the youth to continue living at home, going to school or to work and avoiding arrest or re-arrest. Therapists are available 24 hours a day, seven days a week, wherever and whenever necessary. Therapists work with no more than four to six families at a time. Treatment is intensive, including daily or weekly multiple contacts with family members and others. -Functional Family Therapy (FFT) is a family-focused evidence-based program that emphasizes family strengths to support the development of positive and productive family interactions. Therapists focus on family engagement and motivation, communication and interaction skills, problem solving strategies, conflict management and the use of community resources. Therapists are available for flexible appointment scheduling seven days a week. Therapists can work with 10-12 families at a time. The team supervisor may carry up to five cases. Sessions are generally weekly and will vary with schedules and needs of key family members. Evenings and weekends by appointment.
